Request a document
Raise a request for a specific document through the Westpac App or Online Banking.
Overview
You can use this form to request documents that are unavailable online. Requested documents can be from accounts that have been closed less than seven years ago.
If you need multiple documents, please submit a separate request for each document.

What can’t you request?
Using this form, you won’t be able to request:
- Statements for closed accounts that are available through the Statements section in the Westpac App or Online Banking
- Account statements for a period outside of your usual statement cycle (e.g. monthly or half yearly)
- Documents for a customer who is deceased – for assistance with these documents, please access our Bereavement & Deceased Estates: Step by step guide

Accessing your document
Once your document is ready, we’ll email you with instructions for accessing it. You’ll have seven days to download the document from Share files in the Westpac App or Online Banking, after which time you’ll need to submit another request.
