Your premiums may be reduced if you have stopped smoking for at least 12 months. You will need to fill out and submit a Smoking Questionnaire and your request to alter your smoking status will need to be approved. Call 131 817 (8am-6.30pm, Mon - Fri) for a copy of the form. The premium reduction if approved will take effect from the date that your request has been approved and  notified to you by the insurer.
