What do I need?
Loss of income

A regular benefit if you can’t work due to illness or injury.

To make a claim under either Income Protection or Bill Cover, you should call us with details of your claim for a tele-claims assessment over the phone.

If we are unable to make a decision over the phone, you will generally need to provide:

  • An initial claim form (which we will send to you) that includes sections for you, your treating doctor and treating specialist to complete. The treating doctor’s statements provide details of your condition and your medical history
  • Certified proof of identification
  • Copies of medical reports and investigations performed.
Suffered trauma

Trauma cover pays a one-off amount so you can focus on recovery - rather than your finances.

To make a trauma claim under either Westpac Well or Living Insurance, you will generally need to provide:

  • An initial claim form (which we will send to you) that includes sections for you, your treating doctor and treating specialist to complete. The treating doctor’s statements provide details of your condition and your medical history
  • Certified proof of identification
  • Copies of medical reports and investigations performed.
Suffered disability

Disability cover is your financial back-up plan providing a one-off payment if you’re totally and permanently disabled under the policy definition.

To make a disability claim as part of the Optional Disability Benefit under Lifetime Protect or Total and Permanent Disablement, you'll generally need to provide:

  • An initial claim form (which we will send to you) that includes sections for you, your treating doctor and your treating specialist to complete. The treating doctor’s statements provide details of your condition and your medical history
  • Certified proof of identification
  • Copies of medical reports and investigations performed.

Suffering a terminal illness

Life cover can provide a one-off payment if you are diagnosed as terminally ill.

To make a terminal illness claim under an Instant Cover, Lifetime Protect, Estate Plan or Term Life policy, you'll generally need to provide:

  • An initial claim form (which we will send to you) that includes sections for you, your treating doctor and your treating specialist to complete. The treating doctor’s statements provide details of your condition and your medical history
  • Certified proof of identification
  • Copies of medical reports and investigations performed.
Death

Life cover provides your family with a one-off payment to help secure the future of your loved ones.

To make a death claim under an Accidental Death, Instant Cover, Lifetime Protect, Estate Plan or Term Life policy, you’ll generally need to provide:

  • A completed claim form
  • The original policy document and policy schedule
  • A certified copy of the deceased’s birth certificate
  • A certified copy of the deceased’s will (if available)
  • Probate or letters of administration for policies with a sum insured of more than $50,000
  • If the benefit is under $50,000 we may require a Statutory Declaration, Life Insurance Act 1995, Section 211.
