Your income details
1. Your monthly salary figures (after tax).
To calculate your monthly income, multiply:
- Your weekly salary (after tax) by 4.2
- Your fortnightly salary (after tax), by 2.1.
2. If you are self-employed, income figures from your last two years, tax returns and assessment notices (before and after tax).
3. Your salary or employment number.
4. Your current employer’s details:
- Name
- Address
- Telephone number.
5. If you have not been in your current job for three years, the details of your previous employment:
- Name

6. Your current driver’s licence number and expiry date.
7. If you have not been living at your current address for at least 3 years, the details of your previous address.
8. Details of your assets and liabilities, including bank accounts, shares, life insurance, motor vehicles and personal effects.
Your personal details
9. If you have a mortgage, or have recently applied for a mortgage, the details of the monthly payments.
10. If you are renting or boarding, the details of your landlord / real estate agent including:
- Name
- Contact number.
11. Details of all your monthly financial commitments, such as payments made for other loans, or credit cards.
