The following information is designed to assist you in managing changes to your BT Investor Choice Funds.


Adding to your investment or investing in a new fund
If you have a BT PIN or if you have a current regular investment plan in place complete the following form:

If not, complete the application form from the current BT Investor Choice Product Disclosure Statement.


Accessing BT Link and BT Online
BT Link is an automated phone service that allows you to access personal information and make transactions. BT Online is a secure and easy to use site, allowing you to view your personal information and make transactions. To use these electronic facilities, you need to:


1.

Download the application form from the current BT Investor Choice Product Disclosure Statement.

2.

Provide your BT Investor Number at section 1.

3.

Complete section 11, the 'Direct Debit Request'.

4.

Complete section 12 to allow us to make payments to your nominated account.

5.

Read the terms & conditions relating to BT Link and BT Online and sign the declaration at section 16.

6.

Mail the form to:

Updating your details
To update your contact details (address, email, phone numbers), change your name on our file, update your distribution payment option, provide or amend your bank account details or provide your TFN or ABN complete the following form:

Make monthly payments to your investment
a) through payroll deduction:
Before applying for this facility, confirm that your employer can arrange to have your salary paid to multiple bank accounts. If not, this facility cannot be used.

b) by authorising a directly debit:
This form allows you to add a regular investment plan to any fund. If you have a BT PIN or if you have a current regular investment plan in place, complete this form:

If not, please complete the application form from the current BT Investor Choice Funds Product Disclosure Statement.


Switching between BT Investor Choice Funds
If you have a BT PIN or if you have a current regular investment plan complete this form:

If not, please complete the above form and an application form from the BT Investor Choice Funds Product Disclosure Statement.


Making a withdrawal
To make a partial or full withdrawal, simply complete the following:

Adding a Flexi Payment Plan facility
The Flexi Payment Plan allows you to receive a regular, pre-nominated, payment each distribution period. To set up a Flexi Payment Plan, complete this form:

Providing authority to act on your faxed instructions
Under ordinary circumstances we will only accept instructions received by mail or faxed from a Westpac branch (with branch stamp). This form allows you to provide us with the authority to act on you instructions from any fax.

Appointing an authorised representative
This form will allow you to appoint a person with the legal capacity to act as your authorised representative and to operate investments on your behalf.

Using the Phase-in Plan
The Phase-in Plan gives you the flexibility to "drip feed" your money from the Westpac Cash Management Trust to any other BT Investor Choice Fund over a period of time. If you have a BT PIN or a current regular investment plan, complete the following form:
